Apply to a Fellowship

Addiction Medicine

The Swedish Addiction Medicine Fellowship accepts applications through ERAS and participates in NRMP’s medical and pediatric specialties match. For letters of recommendation, three is traditional, but we require a minimum of two. If you will be coming to fellowship straight from residency, we require that one of the letters be from your residency’s program director.

Sports Medicine

The Swedish Sports Medicine Fellowship accepts applications through ERAS and participates in NRMP’s sports medicine match. The fellowship only considers applicants trained in family medicine or PM&R.

At a minimum, the fellowship requests the following information from applicants:

  • Completed ERAS application

  • 3 letters of recommendation (one from your residency's program director)

  • Curriculum vitae

  • Personal statement

  • Copy of USMLE/COMPLEX transcript

  • Copy of medical school diploma and transcript

  • Certificate or other validation of all previous training

  • Copy of current state medical licensure
